How to Apply

Complete the Participant Application: Online Application (see below) or Participant Application View PDF (to print a paper copy)

Application Checklist

1) Student Portion of web application

  • Are all appropriate fields filled in completely (if a question does not apply to you, please enter "none")?
  • Personal Statement
    • Did you complete your personal statement in the exact format requested?
    • Did you answer all of the questions and complete all five paragraphs?
  • Essay Prompt
    • Did you complete your essay in the exact format requested?
    • Did you provide a complete answer to the prompt?
  • Did you complete the Agreement section of the application and type your name in the "Signature" field?

2) Parent/Guardian Portion of web application

  • Are all appropriate fields filled in completely?
  • If your parents filed an income tax return last year, did you send a copy to or 660.562.1631 (fax)?
  • If your parents did not file an income tax return last year, did you send a signed statement that you did not file income taxes in 2020, which summarizes your financial situation?
  • Did your parents/guardians complete the Consent & Agreements section of the application and type their name(s) into the "Signature" field? Includes:
    • Consent for Medical Treatment
    • Media Release Form
    • Northwest Missouri State University Waiver of Liability Agreement

3) Counselor Recommendation Form View PDF

4) Faculty/Teacher Recommendation Form View PDF

5) Did you save a copy of all portions of the application for your records?

If the answer is "yes" to all of these, you are ready to submit your application! You may turn all application supporting documents (Recommendation forms and Tax documents) to your high school’s school counselor OR send it directly to the Northwest Missouri State University TRIO Upward Bound Office at 372 Administration Building, 800 University Drive, Maryville, MO 64468, or fax it to our office 660.562.1631.

Note: All applicants must also attend at least one field tutoring and one Saturday Academy (by December 4th to complete participant application. Dates available for visitors to attend: September 25, October 16, November 20, and December 4. Below is the bus schedule to assist in transportation to the Northwest campus. Applicants must also ask their school counselor and a faculty/teacher to complete a recommendation form.

Bus Schedule

Busses typically stop at Hy-Vee/Taco Bell after the Saturday Academy from 12:45-1:15 p.m.

Recruitment Visits

The Upward Bound Central Staff has scheduled a recruitment visit to each school.  Any student is welcome to attend and learn of the opportunities through Upward Bound. Below is the presentation given. Please feel free to contact Upward Bound at 660-562-1630 with any questions.
