Requesting an Official Transcript

Transcripts are not issued to individuals who have past due financial obligations, assessment holds or an incomplete admissions file.

Northwest Missouri State University issues transcripts for the following schools:

Northwest is the repository for the transcript records of the other two schools, which are no longer in operation. Click above for a transcript request form for the selected school.

Complete the transcript request form (MS Word document), print and sign it, and mail or fax it to the address below along with your payment.  Information is on the form.
