Applications and Instructions

Completing the Application: Student Instructions

To complete an application for student teaching or residency practicum, please follow these instructions.

  1. Complete one of the following applications, according to whether you plan to complete: a one-year residency practicum and student teaching experience for Elementary certification; a one year residency practicum and student teaching experience for Special Education certification; or a one semester student teaching experience for any other teaching certification. You will need to login with your Northwest username (an "S" followed by 6 numbers) and password.
  2. Complete the form in full. 
  3. Submit the latest version of your resume and degree audit* in PDF format only**
    • *Your degree audit is now available on Bearcat Login
    • **You must use Firefox as your browser, not Google Chrome, to download your degree audit
  4. Save progress or submit form as needed
  5. This form will be reviewed by
    1. Your Advisor > the Registrar’s Office > the TESS Office > Field Experience Office
  6. If it is rejected
    1. You should receive an automatic email and be emailed by whoever rejects it
    2. Please use the link provided in the email to go back into the form, make changes, and resubmit

Reviewing the Application

Please use the Dynamic Forms system to review and either approve or return student teaching and residency practicum applications.

These applications will be reviewed online by each student’s advisor, the Registrar’s Office, the TESS Office and the Field Experience Office. So, all advisors who have a student beginning their residency practicum experience or student teaching in the Fall of 2021, please review their application using the following instructions. 

Here is a brief overview of the instructions:

  • You will receive an email from this address: forms@nwmissouri.edu
  • Click on the link that says “Click here to complete your section of the form.” 
  • Login using your username and password (just like the old version of MyNorthwest)
  • Review the application and any attached files
  • You can then approve the form by signing and hitting the submit button. Or, if there is a problem with the application, you can decline to sign the form by doing the following: 
    1. Click the “Return for Revisions” button on the form;
    2. When you click the return button, an automatic email will appear that allows you to send a secure email message and you can explain the reason for your return of the form to the previous co-signer or to the person who submitted the form.  Please make sure the Subject heading indicates why you are returning/rejecting the form; and
    3. You would then just exit the form at that point without signing it.  This email will be sent to person’s Northwest email with a link that allows the specific form in question to be edited.
