Submit an Alert Form

Northwest Success 360 will be down throughout May 2021 for an upgrade.

University Faculty, Staff or Students are encouraged to use this form to submit concerns, alerts and information. There are no restrictions on what the form is to be used for and may include any information from attendance or other class concerns to behavior concerns to Covid-19 notifications.

Submissions are sent to and reviewed by professional staff to determine next steps. When necessary, instructors and advisors will be notified of pertinent information via email. If you have questions, please contact the Student Success Center at 660-562-1695.

If you have immediate concerns about a student’s health, safety or wellness, please call University Police for immediate assistance at 660-562-1254.

reasons for submitting an alert:

  • General Concern or Notification
    Use this option if you have a general concern about a student or need to notify the Student Success Center about an issue. 

    Example: A student reported that they need to go home for a family related issue.
  • Course Concern or Information
    Use this option for any course related concerns or information.

    Example: A student has yet to attend your course.
  • Covid-19 related absence or information
    Use this option if a student is contact traced or reports that they have or may have been exposed to Covid-19 and need to quarantine or isolate. Please provide dates and whether the student lives on or off campus, if possible.
  • Behavior Concerns
    Use this option when you have a concern about a student’s behavior. As stated above, contact UPD for any immediate concerns about a student’s health, safety, or wellness.
