Membership

The Northwest Fitness Centers are member only facilities, which offer a variety of memberships including Faculty/Staff FULL, Faculty/Staff Equipment Only, Student, and Community. Alternative passes are available to members: day pass, week, and month.

  1. All memberships will be purchased at the main area (LAC 002).
  2. Members complete a Physical Activity Readiness Questionnaire View Word Doc (PAR-Q).
  3. Members complete a Fitness Assessment View Word Doc.
  4. Members complete a Billing Form View Word Doc.
  5. Members with health and/or physical concerns are required to have a medical release to become a valid member.
  6. Members are required to inform Fitness Center staff of any change in their health, any physical concerns, or medications that may affect their ability to perform any physical activity.
  7. Alternative pass members are required to read the PAR-Q View Word Doc and Billing Form View Word Doc, and agree to the content by signing the day pass log.
  8. Student members must be at least 16 years of age and enrolled at Northwest Missouri State University (guardian signature required).

Injuries

Participation in the Northwest Fitness Center is voluntary. Northwest Missouri State University and the Northwest Fitness Center are not liable for injuries suffered by participants; therefore, all participants are advised to consult a physician and to have proper medical coverage before participating in any form of activity. If you are injured or suffer from an illness, the Fitness Center staff will give any assistance possible. The injured party resumes liability for any transportation and/or medical charges.

Equipment Use

  1. Members are required to follow all posted signs and the direction of staff.
  2. Any abuse of equipment or failure to follow rules may result in the forfeiture of membership and fees.
  3. 30 minute courtesy time limit on cardio machines upon member request.
  4. At no time should equipment be removed from the center.
  5. Members are encouraged to clean equipment after each use.

Attire

  1. Appropriate athletic attire should be worn while in the center.
  2. Members are required to wear a shirt at all times.
  3. Members are required to wear a clean pair of close-toed shoes.
  4. No boots, shoes, or attire is allowed that may damage equipment or raise any health concerns.

Refunds

  1. Will be given to the nearest applicable membership fee, depending on the duration of time and circumstance.
  2. Refunds will not be issued for failure to abide by both Northwest Missouri State University and Northwest Fitness Center rules and policies.
  3. Refund requests must be in writing and addressed to the Fitness Director.
  4. Fitness Director has final say on refunds.
