Complete, print and sign the request form, and mail to the address shown at the top of the form. Requests by mail must include payment by cash, check or money order. Credit cards cannot be used when requesting by mail.
Please verify that a faxed transcript will be accepted as official before submitting the request. If a paper copy is required, you must submit a separate request.
Complete, print and sign the request form and fax to the secure fax number at the top of the form. Faxed requests must include payment by a valid credit card. Credit card information must be completed at the bottom of the form.
Requests may also be made in person at the Office of the Registrar, Administration Building, Room 207. Payments must be made at the time of the request by cash, check or money order.
All written requests must include a complete mailing address. If an incorrect address is provided, the student will be responsible for ordering a new transcript.
Requests for transcripts cannot be made by e-mail or by phone.
A HOLD on your record prevents the release of transcripts. Transcripts are not issued to individuals who have Past Due financial obligations, Assessment Holds or an incomplete Admissions file.
Normal processing time is 1-2 working days. Allow time for receipt by mail. Please allow additional time during peak periods at the beginning and end of a trimester as well as during the Holiday Season in mid-December through mid-January.
