Submission Instructions:

Submit faculty, copyright and item information by completing and submit this form using your Web browser and submit files by attaching to an email addressed to Sharon Samson.

OR copy and paste faculty, copyright and item information into the body of an email with files attached.

  • Media files accepted are: BMP, GIF, JPEG, PNG, TIFF, MP3, WAV, AVI, MOV, MPEG. DOC, PDG, PPT, TXT, HTML, and URL's.
  • The information below must be submitted before items will be processed for EReserve use.
  • Media files listed on this request form should be sent via campus email to Sharon Samson or delivered to the Library Services Desk on an electronic storage device, as soon as possible after completing the form.
  • Items will be removed from EReserve at the end of the current trimester unless otherwise noted (use "remove date" field to indicate later removal).

Copyright:

  • During the first trimester material is on reserve, the library staff will attempt to obtain copyright permission for materials that will be placed on reserve for additional trimesters. If they are not able to obtain copyright permission, faculty will have the responsibility for obtaining that permission.
  • This material may be protected by Copyright Law (Title 17 US Code). I understand that the responsibility for compliance with the copyright law rests solely with me, the faculty member who has placed photocopied materials on reserve. (For more information on copyright click here)
