Northwest provides two optional vision insurance plans through Ameritas Group Insurance Company...a Basic/Low Eye Care Plan and High Eye Care Plan. Employees will be responsible for 100% of the premium cost. Employees also have the option of purchasing coverage for dependents. Deductions can be made on a pre-tax basis using the Missouri Cafeteria Plan.
The Basic/Low Eye Care Plan is a reimbursement plan covering the eye exam and glasses or contact lenses. You have the freedom to select any vision provider. When you visit your vision provider, you will pay for the services in full and submit a claim form to Ameritas for reimbursement.
The High Eye Care Plan uses a network of providers under the VSP Network providing coverage for an eye exam and glasses or contact lenses. Employees will pay a copay for the exam and glasses/contacts.
New employees have the initial enrollment period of 30-days to enroll in the vision insurance plans. Open Enrollment is held during November and December with a January 1 effective date.
