Northwest provides Basic Life and Accidental Death and Dismemberment coverage through The Hartford Life Insurance Company in the amount of one (1) times the annual salary. This benefit is available to all benefit eligible employees at no cost to the employee.
If you qualify for Basic Life, you may also apply for Additional Life coverage to supplement your Basic Life amount. You may election coverage in amount of one times your annual earnings to a maximum of $150,000, two times your annual earnings to a maximum of $300,000 or three times your annual earnings to a maximum of $450,000.
New employees have the initial enrollment period of 30-days to enroll in the Additional Life coverage. Open Enrollment is held during November and December with a January 1 effective date.
Late applications and requests for coverage increases are subject to medical underwriting approval. During open enrollment, employees who are currently enrolled in the Additional Life plan can elect to increase coverage up to the next level without submitting medical evidence.
Dependent Life Insurance is also included in this plan. You may elect Dependent Life insurance for your spouse and eligible child(ren). This coverage is available in amounts of $10,000 for your spouse and $5,000 for your child(ren) or $20,000 for your spouse and $10,000 for your child(ren), and you may elect only one from these options. Late applications and requests for coverage increase will also require medical underwriting approval.
Under this plan, coverage reduces by 35 percent at age 70, 55 percent at age 75, and 70 percent at age 80. Coverage on children will terminate when they are no longer your eligible dependent.
For employees who would like to increase their life insurance coverage and have not yet elected coverage, a Personal Health Application will need to be completed. Mail the completed form to The Hartford for medical underwriting approval.
