Auditions are scheduled on a first-come, first-served basis. Please indicate your order of preference for an audition date: ("1" = first choice, "2" = second choice, etc.) and location (when applicable).

Deadline for all auditions is Saturday, Feb. 25, 2012.

Applications will be accepted after January 27, but no guarantee of date preference, audition, or an award.

None of the above. It will not be possible for me to audition on one of the above dates; therefore, I would like to request an audition on .
(Date must be Monday through Friday, 8 a.m. to 5 p.m. and before Feb. 25, 2012.)

The travel distance will prevent me from coming to Maryville to audition; therefore, I would like to submit my audition on a cassette, CD or DVD. (Please send your recording and application form to the address show on the back. Make sure your recording is labeled with your name and address. Audition tapes/CDs will not be returned. Recorded auditions must be received by Feb. 24, 2012 -- no mail delivery on Saturday).
