Limited Accident Insurance Coverage

Northwest has purchased an insurance policy providing limited accident protection for students participating in Intramural Program and Club Sports activities. The participant's personal insurance coverage is the primary payer on the claim; this coverage is secondary. Completed claim forms are to be returned to the Lamkin Recreation Center or Club Sports office.
